In my opinion project teams have to define criterias and benchmarks for estimate.
This will mean, esnuring that some fault levels are in place, one of them could be Unit Testing. Unit Testing prior to submitting code to QA, will give better assurance that SQA can execute at least one full test cycle without encountering true showstoppers.
Furthermore Iterative/Agile development addresses this issue in many cases.
